One (out of 4) Hikvision cameras doesn't work on Unifi USW-16

Moved houses and took cameras with me. New setup is full Unifi, including a 150w USW-16.

3 of my POE hikvisions installed with no problem, but the 4th will not work.
The switch supports 802.3af, which is listed on back of camera.

Camera model: DS-2CD2042WD-I

It will, however, work fine off an unmanaged switch using an injector. The injector is 48v.

What am I missing? I removed some of the other cameras to ensure I wasn't over powering the port, as well as tried multiple cables to be sure.

All 4 cams the same model? I know the 150w USW-16 model has issues with Passive POE. Many articles about the issues with passive POE devices (ironically many Ubiquiti devices utilize passive POE). I had to send a switch back because of it. I ended up buy 2 of the UniFi Switch 8 POE-150W so that I didn't have to deal with any issues with passive devices I have.
