You should be able to hear the audio being captured by the camera when you click speaker icon with out having to click on the mic icon. You only click on the mic icon to enable two-way audio back to the camera. Anyways I figured it out the problem with the help of some peoples suggestions...

Just upgraded my firmware to 5.2.3 and I still have the same problem. The issue is almost exactly as described in the link you provided except I am can enable the speaker icon via live view but still don't get audio unless I enable the mic just like what is happening in iVMS-4500.

Oh I had some hope for a sec but verified that my audio encoding is set to G.711ulaw and I just tested with some of the other options but still no audio output in iOS unless I enable the Mic.

Were you every able to figure out this issue? I am having the same problem, I get no audio unless I enable the mic and there was even a iVMS-4500 app update for IOS but it did not resolve the issue.
