Circling back to this - I installed VLC and connected to the doorbell's RTSP stream at rtsp://<doorbell IP>/Streaming/Channels/101/ - and was able to hear audio through VLC. So seems it might be a QNAP QVR Pro specific issue - either not recording audio in general with Generic RTSP camera...
