With the above settings I get 4 images in the email:

1st - 1536 x 864
2nd - 853 x 480
3rd - 1536 x 864
4th - 1536 x 864

Just trying to figure out why the 2nd image is smaller than the rest. More annoying than a real problem, and it has been this way since I first started with Blue Iris about a year ago, so it not new.
 
I suspect the 2nd image is a database thumbnail.
Suggest an experiment to verify... set up the the trigger to store a hi-res alert image. It should be sent instead.
Once verified, remember to uncheck the option to avoid saving unneeded full-res alert images.

I made that change on one camera and then did a trigger now. This time I got 3 pictures instead of 4 and now the 1st image is smaller!

1st - 853 x 480
2nd - 1536 x 864
3rd - 1536 x 864
 
Was this a manual trigger?
Try waiting for an actual motion trigger...
EDIT: And verify hi-res alert images are being created in your Alerts folder
 
I did a manual. I just got an actual and it was the same as the original where the 2nd image was still small out of the 4.
 
Seems like a dumb thing to contact support about, he's probably got bigger issues to deal with!
 
OK, changed one camera to use the sub-stream and now the images from that one are:

1 - 342 x 192
2 - 856 x 480
3 - 342 x 192
4 - 342 x 192
 
856 x 480 is also the size of image I get when I use HTTP command /thumbs/{filename}

It appears the same-size database thumbnail image is always returned with the option

regardless if a high-res alert image is saved with the trigger, or substream is used.

Answer from Ken about the sizes:

"The 1st one is a pre-trigger image
The 2nd image is from the Alerts list.
Additional are "real time"

The alert image list is not stored as full-size, it's reduced for the database."
