Can't login to NVR after upgrade

I think I might have royally screwed up. I am not able to gain admin access to my NVR. I have a LV-N9808C8E and I was trying to upgrade it to V3.4.93 build 170821. After it successfully upgraded, it messed up my login credentials (see pic 1) and now I can't gain access to my NVR. I tried to reset the password by clicking on "Forgot pass..." but it pulls up a window (see pic 2) to import a file? I'm not sure how to use this to reset it. It doesn't even allow me to change username, they are all pre-selected (see pic 3). I tried to follow the instructions I found online about getting the window with serial number and secure code, but my system does not seem to have that available. Please help.

LV-N9808C8E

LaView sold a lot of Hikvision OEM kits .. so I would expect this to be a Hikvision OEM NVR

I got exactly the same problem trying to upgrade to V3.4.93 build 170821. Could you share how you fix your problem?
