I had a friend who turned a full sized forklift over and he was ejected from the cab and it fell on his head. He didn't survive. Very dangerous uncaged as the centre of gravity seems to be quite high.
As mentioned by others, bald tires, not centered, going uphill, etc. but this one struck me too....if he had no choice about the incline, he could have driven up the ramp headfirst (forward) with the weight of the loader leading, maybe made it OK with the over-center weight. It'll likely be offloaded when he gets to his next stop anyway.
Either way he made too many bad decisions. Uphill loading is dumb. Off center on the ramps is just plain stupid, plus he wasn't even going straight up them in the first place. In too much of a rush IMHO and it cost him bucks for that flatbed.
