NVR Keeps Switching To Default

Hello all. At our storage facility we have a Honeywell NVR, model HEN16162. It is set up to record on motion detect and has been working fine for several years.
Over the past several months, I have found that it will switch to the default recording which is simply recording 24/7.
I have searched and searched within the system settings and cannot figure out why this keeps happening.
Any suggestions?
One other thing which is probably not related. The HonView Touch app on my iPhone has stopped displaying cameras. I can still get logged in but the various channels will not display the camera views. The spinning indicator in the middle of the screen just keeps going and going.
Likewise, any suggestions in this regard?

Is the device remotely accessible from the internet?
Is the device widely accessible on a large, open, internal network?
How many users are defined for configuration access, and do the system logs show any 'interesting' behaviour?
Suggestion :
Change the admin access passwords to see if that changes the problem.
